STEP ONE
YOUR CASE MANAGER RECOMMENDS RESIDENTIAL
Your CSB worker or case manager has identified that your child needs residential treatment. Don’t have a case manager? Click here to find your local CSA Coordinator.
STEP TWO
FILL OUT OUR REFERRAL FORM
Fill out our online referral form here. Please have your child’s insurance information handy when filling out the form.
If you have private insurance: Pre-authorization is required for private insurance. Call the number on the back of your insurance card for more information.
If you have Medicaid: Medicaid has a pre-authorization process called IACCT (Independent Assessment, Certification, and Coordination Team). You can fill out an IACCT inquiry form here. To learn more about the IACCT process, click here.
STEP THREE
OUR ADMISSIONS COORDINATOR WILL CONTACT YOU
Once we assess all funding and review the documentation provided, our Admissions Coordinator will reach out to schedule an interview or request additional information.
STEP FOUR
AN INTERVIEW WILL BE SCHEDULED
Our Admissions Coordinator will schedule an interview and tour of Child & Family Healing Center.
STEP FIVE
DECISION ABOUT PLACEMENT
After your child’s interview and tour, a decision will be made about placement within 1-2 business days.
